DATIpilot Modul 1 Innovationssprints; Einen Pulsschlag voraus: Datengetriebenes Frühwarnsystem in der Kardiologie - PULSE

Abstract

PULSE entwickelte ein datengetriebenes Frühwarnsystem zur Vorhersage schwerwiegender Komplikationen nach elektiven kardiologischen Interventionen (Koronarintervention, TAVI, Vorhofflimmern-Ablation). Aus Basis administrativer Informationen und digitalen 12-Kanal-EKG wurden mehrere Modellarchitekturen trainiert und intern validiert; ein multimodales Ensemble aus einem im Rahmen des Projekts weitertrainierten EKG Foundation Modell und einem auf strukturierten Daten trainierten Modell schnitt am besten ab. Ein Inferenzdienst mit standardisierter, FHIR-basierter Schnittstelle wurde entwickelt, in die kardiologische Arbeitsplatzsoftware MediConnect integriert und getestet. Ergebnis ist eine frühe Produktversion, sowie ein übertragbares Kooperationsmodell zwischen Klinik und Hersteller. PULSE developed a data-driven early warning system to predict severe complications following elective cardiovascular procedures (coronary intervention, TAVI, atrial fibrillation ablation). Several model architectures were trained and internally validated using basic administrative data and digital 12-lead ECGs; a multimodal ensemble consisting of a fine-tuned ECG foundation model and a model trained on structured data performed best. An inference service with a standardized, FHIR-based interface was developed, integrated into and tested with the cardiology software MediConnect. The result is an early product version, as well as a transferable collaboration model between the hospital and the manufacturer.
